We collect basic information such as your name, address, telephone number, date of birth and whether you are a Carer or a Consumer and other details as directed and required by our funding body (Department of Human Services). The information we collect from you is collected for statistical purposes to provide information to the Department of Human Services for the monitoring and assessment of performance of the services offered. All information is confidential and names and addresses etc. are not supplied as part of the reporting processes. We may use your information with your consent to conduct research or improve our service delivery or for the Mood Support Group to recognize service gaps. However, we will use non-identifying information supplied by you in such cases.

WHO WILL USE THE INFORMATION

Your personal information will not be disclosed without your consent unless required by law or a legal authority. Only authorised employees of the Mood Support Group will have access to your personal information once consent has been granted from you. The Mood Support Group will not sell or trade your personal information.
